Has anyone ever had a Tier 4 or below flee from them even if all the throws are perfect?
Everyone basically had legendaries flee from them, but has anyone ever run out of balls to catch a T4 or below and none of the balls missed and all of them are greats/excellents?
The only T4 or below that fled from me are 2 tyranitars but that's because they attacked the balls without a window to throw.

Yes. At least 3 or 4 Tyranitars since I learned to always land all throws on them. I was always using Pinaps for the first throws though.
Also at least 1 Machamp where I landed all throws. Also plenty of failed catches for lower tier raids where I didn't land all balls, 1-2 Machamps, 1 Muk, 1 Charmeleon, probably other things I don't remember.

This actually does not happen. There is always a window, albeit at minimum it is very very small. But it is there and you will hit it if you time your throw correctly.
During a double candy event we went out hunting 3rd evo’s to pinap for 40 candies. I had an Alakazam run after 13 great curve balls. It really put a hurt on my pinap supply.
On Friday I duoed the last Charizard to spawn in town. I caught it on my thirteenth ball, though I switched from pinap to golden razz with 7 balls left.
